The visiting PT and nurse's assessment was that being at home put me in danger. After trying to get BCBS to agree to some sensible course of action, such as sending me back to the SNF, the PT called me last night and essentially said Get thee to the ER.

So here I am. And I just got word that my X-rays don't show any new harm — but that, since I'm clearly not safe at home, they're admitting me. So I'm feeling hopeful for the first time in days.

I'm glad that you are back in the care system and dearly hope that this time they keep you in some fully staffed facility until you are mobile enough to be safe in your own home.

Aside from all the runarounds with your insurance it also seems completely unreasonable that the transition to home - visiting nurse / PT coverage was not better able to catch that. Not safe is bad enough but nobody even assessing what your needs were and seeing for themselves right away... arrrgh. I am so mad on your behalf. But very relieved that you are back in a place where it is safe to recover.

I'm also really glad that the x-rays don't show new physical harm to the surgical site.

You need a lawyer as much as you need healthcare, in order to be sure your "insuror" doesn't ditch you at home again.
