You probably don't give a damn what guys like me think, but on the off chance you do: as far as guys like me are concerned, there is no bigger turn-off than conspicuous stupidity.

Part of me wants to say if she falls everyone should walk by her laughing and maybe she'd learn. Sadly, I'm nicer than that.

I actually wonder whether women who wear heels really care what men think. Sometimes it seems to be a self-indulgence thing, where the pain and reduction in ability to do normal things are not important because the woman feels... er... whatever she feels. Sexy? Empowered? More significant because taller? I find that very weird. Women's magazines are full of articles about how 'we all' have a fetish for 'great shoes' (=high-heeled implements of torture), and how in order to feel 'confident' 'we all' want to wear 'killer heels'. No it's partly a con by the manufacturers and advertisers, but not entirely: functional, attractive-looking shoes for women do exist yet many women still choose to dress their feet as you describe. I think the deeply-held belief that long legs are sexier than short legs is part of it. Do all cultures share that belief?

My intuition is that finding long legs attractive is a secondary effect. High heels change the attitude of a woman's butt and back, in a way that's reminiscent of how a woman may arch her back when she's aroused. This tends to get a straight guy's attention. Of course, in addition to producing this appearance of arousal, heels also produce an illusion of longer legs; I think guys consequently tend to associate long legs with arousal, but aren't necessarily attracted to long legs per se.

As to whether women in killer heels really care what men think: I've been told any number of times (admitedly, mostly by guys who thought they were great experts on women) that women don't dress for men, but rather for one-another. And I've overheard enough clothes-talk among women over the years to know there's a lot of truth to that claim. But that clothes-talk has included enough phrases like I see you're trolling for a new boyfriend for me to find it hard to believe that a grown woman can dress provocatively without some expectation of male reaction.
